Nursing
Our nurses are highly skilled RNs and LPNS that play an extremely important role in providing care and education to the patients and families. Our priority is to help keep patients out of the hospital and at home in a safe environment. Our nurses provide assistance with disease control and health management, pain management, wound care, ostomy care, IV therapies and they will guide you through each visit to ensure you are comfortable, confident and knowledgeable of your plan of care.
Therapy
Our therapy team is comprised of highly qualified clinicians dedicated to providing comprehensive rehabilitation services aimed to meet the unique needs of each individual under our care. We enable you to face and overcome your environment’s natural obstacles, thereby preparing you for a return to normal daily functions and a better quality of living. Our therapy services include:
Physical Therapy
Diagnosis education, disability assessment and evaluation, fall prevention, balance and coordination improvement, transfer training, gait training, strength training, individualized home exercise program, pain management.
Occupational Therapy
Activities of Daily living training and support, postural training, home safety assessment and education, individualized home exercise program, strength training.
Speech Therapy
Receptive and expressive language disorders, Cognitive-communication disorders, Speech disorders, Oral-motor skills, Swallowing disorders, voice disorders, treatment with augmentative-alternative communication systems and devices
Social Workers
Our social workers bring compassion and understanding to your home. They understand the burden that illness and injury can bring and are able to deliver individual and family support by providing guidance to locate appropriate community and social services resources.
